Problem with new netgear fa310tx

I am having the infamous SIOCSIFFLAGS problem with a Netgear FA 310TX
purchased very recently--two weeks ago.  I have read several of the
archived messages concerning this card, so I am aware that there are
issues with the recent use of the Lite-On chip.  However, when I look
at the information presented me from my kernel logs, proc, and
tulip-diag, to my untrained eye the information seems to be different
that what I should expect.  

I have tried various versions of tulip.c, including the version that
shipped with the card and version 0.90, without success.  My system is
running the 2.0.35 kernel, and my failure with tulip.c 0.90 was when
the driver was loaded as a module.  This machine also runs Windows NT
4.0 (with regret), and the card works fine on NT hooked up to a
MediaOne Express cable modem.  

Below is the relevant portion of my kernel log, a copy of my
/proc/pci file, and output from tulip-diag.

I would greatly appreciate any assistance.
