3C905 not detected

I have almost reached the end of my rope with this card.  I am running on a 
pentium 66.  Linux version 2.0.34, 3c59x version .99E

I have tried both with the driver compiled into the kernel, and as a module 
and the card is not detected.  Here is the output from the 
vortex-diag program:  I found it interesting that it seems to detect the card 
twice.  Is this normal?  Any help will be greatly appreciated.

Found a 3Com PCI Ethernet 3c905 rev 0 at 0x4000.
The Vortex chip may be active, so FIFO registers will not be read.
To see all register values use the '-f' flag.
