Tulip working in 2.3.99-pre2-5

As I send all my patches to Donald, he also provides valuable feedback
as well.  So even though he is not directly contributing to 2.3.x
development, he still contributes a great deal in the form of feedback,
and also in the form of code which I merge from his 2.2.x drivers into
2.3.x.

Note there are still two more small fixes that need to go into this
driver WRT the dummy frame DMA handling (or lack thereof), and also WRT
the tx_timeout handling.  And possibly one black magic fix for Alphas. 
(not including all the fixes in 0.91x et al.)

Thanks really go to Dave Miller for finishing the PCI DMA implementation
(which fixes your problem), and to Alexey for spotting the remaining two
issues.
