I have seen the exact same thing with 91g on a 21143-xC w/QSI6611 non-MII
PHY on 2.0.3X.  Only under high load (we have an intensive throughput lab
with 4 100Mb/s Alpha's used as end-points), and always under high load.
Plain 91 dated April seems solid under identical conditions.
