I think I'm having the same problem as you.. somewhere around version 
 >0.79 my network card stopped working with the tulip driver. I found that
if I change the timer routine in the table at the start of the source,
from t21142_timer to tulip_timer, that my network card works again.

My /proc/pci entry is the same as yours (same chip revision).

Note: I know nothing about programming the DEC tulip chip, I just found
this out by comparing the working 0.79 driver with the newer (non-working)
driver. 
I have reported this a few times in the past on this list.. never got any
comments about it ;-(
