My uneducated guess would be bugs in the FTL, I've seen a various
horror stories on the btrfs list about that.

I recently had to update the firmware on a Crucial SSD to fix a bug
that can make it hang after a certain number of hours powered on (not
clear if consecutive or total) - fortunately reported by smartctl when
querying the drive for information.
